Initial Reporting

Patients admitted under Mediclaim must report to the Mediclaim Office immediately upon admission.

Document Submission

All required documents must be submitted personally to the Mediclaim Office within 24 hours of admission. Only after this, the Mediclaim process will begin.

Important Notice

Failure to submit the documents within the stipulated time will result in the patient being treated as a cash patient.

For any queries regarding Mediclaim, please contact our Insurance Department during working hours.

Documents Required for Mediclaim:

Copy of the current year's Mediclaim policy or insurance card.

Copy of the patient’s Aadhaar card and PAN card (Photo ID proof).

All doctor prescriptions/documents, investigation reports, and copies of prescriptions from Bombay Hospital and any external hospitals/clinics.

Note: In case of surgery or operation, if any pre-existing disease is involved, submit copies of Mediclaim policy from the past 3–4 years.
